The following disclosures are required by the federal Electronic Signatures in Global and National Commerce Act ("ESIGN"). They are necessary to enable Waukesha State Bank to deliver required loan disclosure documents to you electronically.
Electronic Delivery of Disclosures and Notices
By returning this e-mail affirmatively stating that you agree to electronic delivery, you are consenting to receive the following disclosures, notices, terms and conditions, and other documents and all changes to the disclosures, notices, etc. electronically. In order to process your request, Waukesha State Bank requires that you consent to the terms and conditions outlined below.
- Some or all of the disclosures, records, or other information (collectively, "disclosures") related to or stated within your loan application will be made available to you in electronic format. Some of these disclosures will not be distributed in paper unless you contact us and request a paper version of a specific record.

- You may withdraw your consent to receive electronically.

- Your consent applies to all disclosures, records or other information related to your application.
- To ensure that we are able to provide you with notices and information from time to time, you must update us with any change in your email address. - In order to receive an electronic copy of the disclosures, notices, terms and conditions, other documents, and changes you must have the following equipment and software:
